Thank you for your time and assistance with the unannounced monitoring inspection conducted today, July 3, 2018. The on-site inspection was conducted from 11:55 a.m. to 5:15 p.m. At the time of arrival there were 83 children in eight classrooms and in the care of 15 staff members with three staff members available to assist if needed. The total enrollment for the program was 124 children. The records for ten staff members were reviewed, and nine children's records were reviewed. There were ten staff members and five children interviewed. The medication and medication paperwork were reviewed for five children. The exception to compliance was noted on the violation notice. Snack was observed to meet the USDA guidelines and consisted of whole wheat tortillas, applesauce, and water. The theme of the week was the Fourth of July. Some items observed and reviewed were: required posted information, emergency, shelter-in-place, and lockdown drills, updated emergency preparedness plan, warm water temperature, injury reports, infant care, diapering procedures, hand washing and bathroom procedures, outdoor water play, snack, nap time, choice time, and dismissal procedures. Standard #: 22VAC40-185-420-E-3 Description: Based on review of nine children's records and verification from the director, the center failed to obtain annual updated enrollment information for three children. Evidence: 1. The record for Child #2 did not contain documentation that the parent checked the child's enrollment information for updates. Date of enrollment for Child #2 was 03/18/2014. 2. The record for Child #6 did not contain documentation that the parent checked the child's enrollment information for updates. Date of enrollment for Child #6 was 11/16/2015. 3. The record for Child #8 did not contain documentation that the parent checked the child's enrollment information for updates. Date of enrollment for Child # 8 was 03/02/2015. Plan of Correction: The staff will have all parents complete updated enrollment information.
