A Subsidy Health and Safety inspection was conducted on 7/5/2018 to review supplemental health and safety requirements. There were 54 children (infants-6yrs.) supervised by 11 direct care staff. The physical plant, 3 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed eating lunch, playing barbies, Legos, coloring, and playing dinosaurs. Diapering and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on record review, 2 of 3 staff records did not contain documentation of completed Virginia Preservice Training. Evidence: Staff A (date of hire: 9/6/2005) and Staff C's (date of hire: 8/1/2014) records did not contain documentation of completed Virginia Preservice Training.

Plan of Correction: We will have all staff complete training.

Description: Based on record review, the center did not maintain procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child. Evidence: During the time of inspection, there was no documentation regarding reunification plans for emergencies.

Plan of Correction: We will add it into our emergency plan.

Standard #: VENDSUB-000-032-A
Description: Based on record review, the center did not practice lockdown procedures at least annually. Evidence: During the time of inspection, there was no documentation of completed lockdown drills.

Plan of Correction: We will conduct a lockdown drill once a year.
