Comments:
An unannounced monitoring inspection was begun on June 26, 2018 specifically for the purpose of inspecting the Substitute pool staff files. The files, while housed at this center, are not under the supervision or responsibility of the director at this center. The record review appears on this facility on as a base of operations. Standard #: 22VAC40-185-160-A
Description: Based upon a review records, one staff record did not contain documentation of a TB screening or test within 21 days of employment. Evidence: Staff # 6 has TB documentation dated 12/18/16. Two possible Dates of Hire (DOH) are listed (6/5/15 and 8/12/16); however neither one is within 21 days of the date of the TB test.

Plan of Correction: A new Sub pool coordinator is in place along with a new system for obtaining documentation as required.

Description: Based upon a review of records, staff records do not contain all required information. Evidence: 6 staff records were reviewed. Missing information includes but may not be limited to: *6 of 6 did not contain a Date of Hire (DOH). (Staff #1-6) *4 of 6 did not contain an employment application/address/phone number (Staff #2,3,5) *5 of 6 did not contain documentation of 2 references (Staff #2,3,4,5,6)

Description: Based upon a review of records, staff records did not contain documentation of orientation by the end of the first day of assuming job responsibilities. Evidence: Orientation for staff available for review did not contain all required components, was not completed within the first day of assuming job responsibilities, or by the end of the first week of supervising children. Missing components include but may not be limited to: Confidential treatment of information and licensing standards relating to their position. (staff #1-6)

Plan of Correction: A new Sub pool coordinator is in place along with a new system for obtaining documentation as required.

Standard #: 22VAC40-191-40-D-1-A
Description: Based upon a review of records, staff records did not contain documentation of a completed Central registry search (CPS) within 30 days of employment. Evidence: Staff #1 's (possible DOH: 3/7/18) search is dated 4/13/18, past 30 days of employment. Staff #3's (possible DOH: 6/24/16) search is dated 8/3/16, past 30 days of employment. Staff #4 (possible DOH: 9/21/15) does not have an original document on file. The form available is a photocopy, and dated 1/23/17. Staff #5 does not have an original document on file, only a photocopy date 1/10/17. No DOH is available. Staff #6's (possible DOH: 6/5/15) document is dated 7/10/15, past 30 days of employment.

Plan of Correction: A new Sub pool coordinator is in place along with a new system for obtaining documentation as required.

Description: Based upon a review of records, staff records do not contain documentation of a Criminal Record Check (CRC) within 30 days of employment. Evidence: Staff #2's (possible DOH: 4/5/17) CRC is dated 5/8/17, past 30 days of employment. Staff # 3's (possible DOH: 6/24/17) CRC is dated 7/29/16, past 30 days of employment. Staff # 4's (possible DOH: 9/21/15) CRC is not an original and is dated 2/1/17, past 30 days of employment. Staff #5's (No DOH documented) CRC is a photocopy, not an original document. Staff #6's (possible DOH: 6/5/15) CRC is dated 7/9/15, past 30 days of employment.

Plan of Correction: A new Sub pool coordinator is in place along with a new system for obtaining documentation as required.

Description: Based upon a review of records, one staff record did not contain documentation of a completed Sworn Disclosure Statement (SDS) or affirmation prior to the first day of employment. Evidence: Staff #6's SDS is dated 6/18/15. Two possible DOHs are listed as 6/5/15 and 8/12/16.

Standard #: 63.2-1720.1-B-2
Description: Based upon a review of records, staff files did not contain documentation of and Eligible for Hire letter prior to employment. Evidence: Staff #1's Eligible for Hire letter is dated 3/9/18. Emergency contact information was dated 3/7/18, with no specific DOH listed.

Plan of Correction: A new Sub pool coordinator is in place along with a new system for obtaining documentation as required.

Standard #: 63.2-1720.1-B-3
Description: Based upon a review of records, 1 staff record did not contain documentation of an Out Of State Child Abuse and Neglect Search for a staff member that has resided outside of the state of Virginia within the past 5 years. Evidence: Staff #3 has resided outside of Virginia, in Maryland, in the past five years. There is no out of state search on file as of September 1, 2017 for this staff member.

Plan of Correction: A new Sub pool coordinator is in place along with a new system for obtaining documentation as required.
