A Subsidy Health and Safety inspection was conducted on July 11, 2018 to review supplemental health and safety requirements. Licensing Inspector arrived at 12:40pm and departed at 3:30pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 30 school age children present with three staff members during the summer camp program. Children were observed playing in various stations created in the cafeteria to include jump rope, dramatic play and blocks. The children were also observed working in small groups constructing houses. The sample size consisted of five children's records. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-006-B-11 Description: Based on review, the vendor did not ensure that there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: There was a diagnosed food allergy in the record indicated for Child #6 and the center had not obtained a written allergy care plan. Plan of Correction: Summer Camp Director will talk to the parent and the MAT trained staff.
Standard #: VENDSUB-000-006-B-3 Description: Based on record review, in four of five children's records reviewed, the vendor did not ensure that all required parent contact information. Evidence: There was no documentation of one or both parent's work addresses in the records for Child #1, Child #2, Child #3 and Child #5. Plan of Correction: Summer Camp Director will talk to all parents and make sure to get the correct address for the files.
Standard #: VENDSUB-000-032-A Description: Based on review, the vendor did not ensure that all required emergency response drills were practiced. Evidence: There was no documentation of a lockdown drill conducted in 2017. Plan of Correction: Summer Camp Director will make sure that lockdown drills are done every year.
