An unannounced monitoring inspection was conducted from 9:30 am - 12:15 pm. There were 72 children in care being supervised by 16 staff members. The children were observed participating in structured center play, outdoor play, and crafts. Lunch is provided from home. Ten children's records were reviewed. Seven staff records were reviewed. Three medications were reviewed and in compliance. There are currently four staff members on site who are Mat (Medication Administration Training) certified. The most recent emergency escape drill was conducted 06/08/2018 and the most recent shelter in place drill was conducted 05/14/2018. Standard #: 22VAC40-185-130-A Description: Based on review of ten children's records it was determined that not all records contained documentation of immunizations before the first date of enrollment. Evidence: Child C with a start date of 05/01/2018 did not have documentation of immunizations on file. Plan of Correction: Administration will request a copy of the most recent immunizations from the parents of Child C. This information has been previously requested.
Standard #: 22VAC40-185-140-A Description: Based on review of ten children's records it was determined that not all records contained documentation of a physical within the first 30 days of enrollment. Evidence: Child C with a start date of 05/01/18 did not have documentation of a physical on file. Plan of Correction: Administration will request a copy of the most recent Physical from the parents of Child C. This information has been previously requested.
Standard #: 22VAC40-185-160-C Description: Based on review of seven staff records it was determined that not all staff repeated a TB (Tuberculosis) screening every two years. Evidence: The most recent TB screening on file for Staff A is dated 04/11/2016. Plan of Correction: Staff A will obtain an up to date TB screening on 07/16/2018.
Standard #: 22VAC40-185-270-A Description: Based on observation of Room #108 it was determined that not all areas were maintained in clean and operable condition. Evidence: Two tables in Room #108 have cracked surfacing with exposed particle board. The particle bard is absorbent and does not allow these areas to be properly cleaned and sanitized. Plan of Correction: Administration will remove these tables from the classroom.
Standard #: 22VAC40-185-500-A Description: Based on observation of the younger infant and older infant room it was determined that children did not always wash their hands after toileting or diapering. Evidence: Child A and Child B's hands were not washed after being diapered in the Younger Infant and Older Infant rooms. Child C did not was their hands after toileting in the the Two year old room. Plan of Correction: Administration will retrain staff to wash children's hands after diapering and toileting using the proper handwashing procedures.
Standard #: 22VAC40-185-500-B Description: Based on observation of diaper area in the Toddler and Two year old room it was determined that not all procedures for diapering are followed. Evidence: The changing area in the Toddler and Two year old classrooms do not have foot operated trash cans. Plan of Correction: Administration will purchase a foot operated trash cans for the Toddler and Two year old room.
