Technical Assistance:
Background check regulations, to include out-of-state central registry searches, were discussed with the center director during the inspection. All staff must complete fingerprint checks by September 30, 2018.

Comments:
An unannounced monitoring inspection was conducted from 10:20am until 1:15pm. At the time of the tour, there were 78 children in care with 11 staff present. A sample of 8 children's records and 8 staff records were reviewed. Children were observed participating in morning activities, playing on the playground, eating lunch and transitioning to nap time. Infant diapering, hand washing and restroom procedures were also observed. Medication, transportation, first aid and emergency supplies, children's injury reports, documentation of emergency practice drills and center postings were reviewed. Violations in the area of administration are identified on the violation notice and were discussed with the center director during the exit interview.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review and interview, the center did not ensure that documentation that each child has received the immunizations required by the State Board of Health was obtained before the child can attend the center. Evidence: 1. Child #3 has a documented date of enrollment of 4/11/18 and was in care on the day of the inspection. Documentation of immunizations was not obtained until 5/22/18. 2. The center director confirmed that documentation of immunizations for child #3 was not obtained before the child entered care.

Plan of Correction: The center responded with the following: Moving forward the center will make sure immunization copies have the prior printed copy dates not after the enrollment dates. We will ensure dates are prior to enrollment.

Standard #: 22VAC40-185-140-A
Description: Based on record review and interview, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: 1. Child #3 has a date of enrollment of 4/11/18 and was in care on the day of the inspection. The record for child #3 contains documentation of a physical examination dated 5/22/18, which is more than one month after attendance. 2. The center director confirmed that the physical exam for child #3 was completed more than one month after attendance.

Plan of Correction: The center responded with the following: Moving forward the center will make sure immunization copies have the prior printed copy dates not after the enrollment dates. Asked parent for original copy and placed in child's file.

Standard #: 22VAC40-185-140-B
Description: Based on record review and interview, the center did not ensure that if a child has had a physical examination prior to attendance, it shall be within 12 months prior to attendance for children two years of age through five years of age. Evidence: 1. Child #4 (age 3 years) has a date of enrollment of 4/6/18 and was in care on the day of the inspection. The record for child #4 contains documentation of a physical examination dated 8/10/16, which is more than 12 months prior to attendance. 2. The center director confirmed that the physical exam for child #4 was completed more than 12 months prior to the child's attendance at the center.

Plan of Correction: The center responded with the following: Mom made an appointment for the child to obtain a new physical. Moving forward the center will make sure to have physical prior to the 12 month window.

Standard #: 22VAC40-185-60-A
Description: Based on record review and interview, the center did not ensure that each child's record contains all of the required information. Evidence: 1. The records for child #1 (date of enrollment: 6/25/18) and child #2 (date of enrollment: 12/11/17) are missing the complete addresses of two designated people to call in an emergency if a parent cannot be reached. Both children were in care on the day of the inspection. 2. The center director confirmed that the records for child #1 and #2 are missing the addresses for the two emergency contacts.

Plan of Correction: The center responded with the following: Moving forward the center will make sure the street addresses are added. Also, make sure to have complete addresses on child's file.
