An unannounced monitoring inspection was conducted during the summer camp portion of the program on July 13, 2018. There were three staff present with 39 school age children and two volunteers. Children were observed having snack playing outside, and doing various tabletop activities to include chess, arts and crafts, and legos.. Five children's records were reviewed on this date. Two medications were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, nutrition, injury report documentation, medication procedures, children's records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, indoor areas, emergency supplies and required postings. Violations were reviewed with and verified by the Recreation Leader.

Standard #: 22VAC40-185-70-A
Description: Based on review, the licensee did not ensure that all required information was at the center for each staff and volunteer. Evidence: During the inspection conducted on July 13, 2018, the following information was not at the center for Volunteer #4: Name, address, verification of age requirement, job title, and date of volunteering; and name, address and telephone number of a person to be notified in an emergency.

Plan of Correction: The staff member completed the form during the inspection.

Standard #: 22VAC40-185-350-E-5
Description: Based on observation and inspection of the facility, the licensee did not ensure that ratios were maintained for children from age of eligibility to attend public school through eight years (one staff member for every 18 children). Evidence: During the inspection conducted on July 13, 2018, there were two staff members on the outdoor play area with 39 school age children, the youngest children in the group were five years old.

Plan of Correction: We will make sure extra staff will be outside with direct sight of children. There were enough staff present on site.

Standard #: 22VAC40-185-510-C
Description: Based on review, the licensee did not ensure that center's medication procedures were followed regarding methods to prevent use of outdated medication. Evidence: During the inspection conducted on July 13, 2018, an emergency medication was observed that expired in February 2018. The center's policy indicated that " staff would have to notify parents/guardians that medications are soon expiring. This should take place within 14 days prior to expiration. If the household has been notified and they have not requested for it back, the center is to complete a Medication Return Slip or a Medication Disposal Slip. Outdated medications should not be left at site locations".

Plan of Correction: We received the medication 24 hours ago prior to a field trip and while moving fast, did not have a chance to check the medication date. The medication will be returned on the child's next day of camp.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation and inspection of the facility, the licensee did not ensure that the findings of the most recent inspection of the facility were posted. Evidence: During the inspection conducted on July 13, 2018, the facility had the findings posted from an inspection conducted on 1/19/18 but the most recent inspection conducted for the center was completed on 3/29/18.

Plan of Correction: Today's inspection will be posted.
