Comments:
An unannounced monitoring inspection was conducted today from 9:30am-1:00pm. There were 29 children (2-5yrs.) supervised by 5 direct care staff. The physical plant, 2 staff records, 3 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed engaging in outdoor play and cooking projects, discussing plants and gardening, and playing dress-up and legos. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-160-A
Description: Based on record review, 1 of 2 staff records did not contain documentation of a negative tuberculosis screening within 21 days after employment. Evidence: Staff B's record (date of hire: 5/10/2018) did not contain documentation of a TB screening.

Plan of Correction: She will get the results on Monday.

Standard #: 22VAC40-185-60-A
Description: Based on record review, 2 of 3 records did not contain documentation of all required information. Evidence: 1. Child #1's record (start date: 8/10/2015) did not contain documentation of child updates and confirmation of up-to-date information in the child's record as required by 22 VAC 40-185-420 E 3 2. Child #2's record (start date: 5/7/2018) did not contain documentation of the addresses of two designated people to call in an emergency if a parent cannot be reached

Plan of Correction: We will update today when parents come to pick up.

Standard #: 22VAC40-185-240-A
Description: Based on record review, 2 of 2 staff records did not contain documentation of completed training by the end of their first day of assuming job responsibilities: Evidence: Staff A (date of hire: 5/2/2018) and Staff B (date of hire: 5/10/2018) did not contain documentation of completed training by the end of their first day of assuming job responsibilities, as required.

Plan of Correction: We will complete it by Monday.

Standard #: 22VAC40-185-510-J
Description: Based on a review of medications, 1 of 3 medications was not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. Child #1's (start date: 8/10/2015) Epi-Pen was stored in a fanny-pack, hanging outside of the kitchen door. 2. The fanny-pack, was zipped, but did not have a locking mechanism.

Plan of Correction: Corrected during time of inspection. I will make sure that we lock it back up after field trips.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, 2 of 2 staff records did not contain documentation of a central registry finding within 30 days of employment. Evidence: 1. Staff A (date of hire: 5/2/2018) and Staff B's (date of hire: 5/10/2018) records did not contain documentation of central registry findings. 2. Central registry requests were mailed 5/30/2018.

Plan of Correction: I will follow-up.

Standard #: 63.2-1720.1-B-3
Description: Based on record review and staff interview, the center did not perform an out-of-state child abuse and neglect registry for the state, outside of Virginia, that the individual has resided in within the past 5 years. Evidence: 1. Staff B's (date of hire: 5/10/2018) sworn disclosure statement indicated that the employee has lived in the state of California within the past 5 years. 2. Staff B's file did not contain documentation of a child abuse and neglect search request.

Plan of Correction: We will make sure to do it moving forward.
