A joint agency inspection was conducted after the center called about an incident in which a 4 year old child was not in sight and sound supervision. Interviews and video footage supported the concerns and the center has been provided with the results of the inspection and interviews. Please call with questions 703-479-4679.

Description: Based on interviews, observation, and record review, a staff member did not ensure the care and safety of a 4 year old child on June 14, 2018, when staff were unaware Child #1 walked into the class alone and was unsupervised for 1 minute and 48 seconds. Evidence: 1. Interviews with a staff determined Staff A did not realize Child 1 walked into the door from the playground and into the classroom. 2. The center has video footage of all classrooms and the video of the incident was shown to the inspector. The footage indicates the time was 615 PM and the entire time Child 1 was alone in the classroom clocks at 1 minute and 48 seconds.

Plan of Correction: Staff is no longer present and the staff were retrained about head counts.
