An unannounced subsidy health and safety inspection (SHSI) was conducted with the center director and center staff. A licensing inspection was also conducted on this date (see that report for details). Reminder: all staff must have documentation of the mandatory VA Pre-service Training within 90 days of hire. Staff must have documentation of first aid and CPR certification within the first 30 days of hire. There are no children with diagnosed food allergies currently enrolled. Evacuation drills have been conducted monthly, shelter in place drills are conducted twice a year, and a lockdown drill has been conducted once a year. Lockdown drills are to isolate children and staff from a security threat; shelter in place is to protect children from a natural or manmade threat. If you have any questions regarding this inspection, please contact me at 540-219-4122 or email firstname.lastname@example.org. If you have specific questions regarding the subsidy program, visit our website at http://www.dss.virginia.gov/family/cc/assistance_providers.cgi
A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
