Comments:
An unannounced renewal inspection was conducted on July 20, 2018 from approximately 10:45 AM to 2:15 PM to determine the center's compliance with licensing standards. The child to staff ratios were deemed in compliance with licensing standards. The census today consisted of 40 children. The following child to staff ratios were observed: 2 year olds: 9:2, 3's and 4's: 10:2, Younger school age: 13:1 and Older school age: 8:1. Conversations were held with children and staff. The staff were observed to be engaged with the children in age appropriate activities and to be meeting the needs of the children in care. Lunch today consisted of diced turkey and ham, salad, diced melon, and milk. The licensing inspector reviewed four children's records and five staff records. All areas of the facility were inspected to include the kitchen, bathrooms, classrooms, playgrounds, and bus. The facility has age and stage appropriate materials and supplies. The facility's license, last inspection, emergency numbers, evacuation maps/procedures, daily schedule, allergy information and various parent information were observed to be posted. The first aid kit and non-medical emergency supplies were inspected and found complete. The last evacuation drill was conducted on 7/17/18. The last shelter-in-place drill was conducted on 7/20/18. The Emergency Preparedness plan is current. Health Inspection: 7/13/18. Fire Inspection: 9/16/17. Medications were reviewed. Multiple staff have current MAT certification, CPR & First aid training, and Daily Health Observation training. The violations cited during the licensing period were checked for corrections. There were no repeat violations during today's inspection. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the licensing office within 5 business days. Your plan of correction should contain 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure that all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. EVIDENCE: A child-sized wooden bench on the playground had broken, splintered wood and exposed screws on the seat.

Plan of Correction: 1. Upon discovering that the bench was in disrepair, the area was immediately closed off to prevent injury. (Immediately)
2. [The] Executive Director, will oversee the removal or repair of the bench. (August 10, 2018)
3. [The] Center Lead Teacher, will add the Outside Classroom to the weekly playground inspection and any items needing repair will be reported directly to [the Executive Director]. (Ongoing)
