Comments:
An unannounced Complaint inspection was conducted this date regarding care and provisions, specifically pertaining to hand washing. The preponderance of evidence found during the inspection supports a valid finding. There was one other violation that was found that was not part of the complaint allegation.

Standard #: 22VAC40-185-500-A
Complaint related: Yes
Description: Based on observation of the center's video recording system, hand washing was not completed as required. Evidence: l. Staff was observed on 07/24/2018 not washing hands or children's hands after a diaper change. 2. Staff was observed picking up another child directly after changing a previous child's diaper and proceeding with changing the second child's diaper. 3. On 07/24/2018, staff did not ensure that children's hands were washed before and after eating lunch. 4. On 07/24/2018, not all staff washed hands before the children ate lunch.

Plan of Correction: Staff have been retrained on how to properly wash, clean, sanitize the diaper changing area, how to properly wash child's hands as well as adults, and on how frequently hands need to be washed.

Description: Based on the center's own video recording system, staff failed to follow all diapering requirements. Evidence: 1. On 07/24/2018, staff were observed turning away from the diaper changing table to dispose of a diaper leaving a child The diaper station table is approximately 36 inches in height. 2. On 07/24/2018, staff did not wash and sanitize the diapering surface after diaper changing.

Plan of Correction: Staff have been re-trained with documentation from regulations 22-VAC-40-185-500 Handwashing and toileting procedures. Management will do frequent Q's and A's about the process.

Complaint related: Yes
Description: Based on observation of the Rainbow Fish classroom diaper ointment was found accessible to children in care. Evidence: On 07/27/2018, Licensing Inspector observed one child opening the cabinet located under the diaper station in the Rainbow Fish class. The cabinet contained topical ointments that were accessible to the children in care.

Plan of Correction: Staff have been trained on locking cabinets when getting or returning supplies.

Description: Based on observation of the center's own video recording system, staff did not sit with children during meal times. Evidence: 1. On 07/23/2018 and 07/24/2018 no staff in the Rainbow Fish or Starfish classrooms sat with children during meal times. 2. Staff were observed sweeping floors and standing together while the children were eating.

Plan of Correction: Staff have been re-trained on the importance of family style dining and sitting with the children during meal times. Management will do frequent walk bys to ensure policy is being followed.
