Discussed: 1. ratios during designated rest periods: the ratios should not be doubled until the vast majority of children are settling down and resting. 2. reassignment of children: A center may temporarily reassign a child from their regular group and staff members for reasons of administrative necessity but not casually or repeatedly disrupt a child's schedule and attachment to their staff members and group.

An unannounced monitoring inspection was conducted beginning at 12:15 p.m. to 2:30 p.m. with the director. There were 43 children in care ranging in age from two to eleven years of age with five staff. School agers were observed during quiet reading time. After quiet reading time today, school agers participated in 'funtastic fitness'. Twos, threes and fours were observed during naptime. The playground could not be inspected due to rain during the inspection. Five children's records and three staff records including two new staff were reviewed. Emergency drills are up to date. Standard #: 22VAC40-185-60-A Description: Based on review of five children's records, not all records were complete. Evidence: Child #1, enrolled 2/7/18, did not have a full address for second emergency contact. Child #5, enrolled 4/3/2018, did not have a second emergency contact other than the parents. Plan of Correction: The information will be requested from the parents again.
Standard #: 22VAC40-185-70-A Description: Based on review of three staff records, the center failed to obtain references before the first day of employment. Evidence: Staff #2's record, start date 6/25/2018, did not have documentation of two references regarding character, reputation and competency before employment. Plan of Correction: This information will be updated in the file. In the future, references will be obtained prior to employment.
Standard #: 22VAC40-185-270-A Description: Based on observation, not all areas of the center were maintained in clean condition. Evidence: The Pre-K class was observed during naptime. There was food all over the table and on the floor under the tables and chairs. Plan of Correction: Staff will be retrained on proper cleaning procedures
