Comments:
A Subsidy Health and Safety inspection was conducted on 7/26/18 from 10:30 am until 1:45 pm to review supplemental health and safety requirements. At the time of the inspection 21 children were present with five staff members. The sample size consisted of five staff's records, five children's records, one child's medication and supporting documents, one child's diaper ointment, and four children's sunscreen. Children and staff were observed during structured play, an art project, bathroom break, hand washing, preparing for lunch, lunch, nap, transitions, and behavioral management. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-006-B-11
Description: Based on record review and interview, the administrator failed to obtain a written allergy care plan from a physician for each child with a diagnosed food allergy. Evidence: 1. The list of allergies posted shows child 3 has a food allergy and the center has an Epipen for child 3. 2. The administrator stated they had not obtained an allergy care plan for child 3.

Plan of Correction: The parents/guardians of child 3 will be advised to provide an allergy care plan from a physician.

Standard #: VENDSUB-000-012-E
Description: Based on record review and interview, the administrator failed to ensure all staff providing care to children have within 30 days of the date of employment a current certification in CPR and first aid. Evidence: 1. The record for staff 4 documents the employment date as 5/21/18 and staff 5 as 5/29/18. Neither record has documentation of the staff completing CPR and first aid training. 2. The administrator stated neither staff 4 or staff 5 have completed first aid and CPR training.

Plan of Correction: Staff 4 and staff 5 are scheduled for CPR and first aid training in two weeks.
