An announced monitoring inspection was conducted on August 2, 2018 to conduct a record review. Seventeen staff records were reviewed. Violations found are noted on the violation notice.

Standard #: 22VAC40-185-70-A
Description: Based on review, facility did not ensure 3 of 17 staff records have documentation of required information. Evidence: On August 2, 2018, the following was found: 1. The record for staff #8 (start date April 29, 2015) did not have documentation of a second reference check. 2. The record for staff #14 (start date April 11, 2016) did not have documentation of two reference checks. 3. The record for staff #15 (start date December 1, 2016) did not have documentation of orientation training although staff report the training was completed.

Plan of Correction: Recreation Coordinator will obtain reference checks. Recreation Coordinator will reach out and request orientation training documentation. If it cannot be located, the training will be re-done.
