22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 7:24am-12:30pm. At the time of entrance, 10 children were in care with 5 staff members present. Children were observed during snack time, listening to music, free play, reading books, artwork activities, a group puzzle activity and circle time. A selection of staff and children records, emergency drill record and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-130-A Description: Based on record review, the center failed to obtain documentation of each child's immunization record before the child attended the center. Evidence: 1. Child #2's (start date 8/20/2017) file was missing the child's immunization record. Plan of Correction: I will ask the parent for a copy of it.
Standard #: 22VAC40-185-140-A Description: Based on record review, the center failed to obtain from each child a copy of the physical examination within one month after attendance. Evidence: 1. Child #2's (start date 8/20/2017) file was missing the child's physical examination. Plan of Correction: I will ask the parent for a copy of the physical.
Standard #: 22VAC40-185-60-A Description: Based on record review, three of five children's records did not have complete information. Evidence: 1. Child #1 was missing documentation of child updates and confirmation of up-to-date information in the child's record as required by 22 VAC 40-185-420 E 3. The last update was signed on 1/09/2017. 2. Child #2 (start date 8/20/2017) was missing the child's health information as required by 22 VAC40-185-130 through 22 VAC40-185-150. 3. Child #3 was missing documentation of child updates and confirmation of up-to-date information in the child's record as required by 22 VAC 40-185-420 E 3. The last update was signed on 12/08/2016. The child was also missing both parent's work phone number. Plan of Correction: I will have the parent's review the information and update and sign it.
Standard #: 22VAC40-185-270-A Description: Based on observation, equipment of the center outside were not maintained in a safe and operable condition. Evidence: 1. One blue dog rocker on the playground had a torn fabric ear attached to the toy. 2. Two tricycles were missing the left pedals and one tricycle was missing both pedals. Plan of Correction: I removed the dog rockers and will have pedals put on the tricycles.
Standard #: 22VAC40-185-550-E Description: Based on record review and interview, the center failed to maintain a record of the dates of the practice drills for one year. Evidence: 1. The center fire drill record was missing the dates of the practice fire drills for March 2018, April 2018 and May 2018. 2. The Assistant Director stated the practice drills were conducted, but the drills did not get documented on their record. Plan of Correction: I will make sure they are documented in the future.
