Comments:
An Unannounced Monitoring Inspection was conducted on August 1, 2018 related to the areas of programs, staffing, background checks and special services. Upon entrance there were 25 children in care and 7 staff present with children. The record review sample consisted of 5 staff records. Children were observed eating lunch, preparing for and during rest period. The results of the inspection were reviewed with the center director. Violations:
Standard #: 22VAC40-185-560-F-4
Description: Based on staff interviews and observation, the licensee did not ensure children three years of age or younger were not offered foods that are considered to be potential choking hazards. Evidence: During the inspection on August 1, 2018, three children age two (child #1, #2 and #3) were observed during lunch and they were served apple slices and apple chunks. The apple slices measured approximately 2 1/2 inch in length and 1/4 inch thick. The apple chunks measured 1 1/2 inch thick. The center staff (staff #1 and #2)confirmed they observed the children eating the apples. Foods that are round, hard, small, slippery, or thick are considered high-risk foods for young children because they can lodge in small airways causing a child to choke.

Plan of Correction: Before food is served director or assistant director will approve the cuts of food items.

Standard #: 22VAC40-191-60-B
Description: Based on record review and staff interviews ,the licensee did not ensure that all employee of a licensed child welfare agency must not be employed until the agency has the person's completed sworn statement or affirmation. Evidence; During the inspection on August 1, 2018, the record for staff #3 (first date of employment 7-30-18), did not include a signed statement of affirmation.

Plan of Correction: The Director will follow up after orientation and go through orientation packet twice to ensure all items are signed. Staff #3 will sign the form on 8-2-2018.
