Comments:
An unannounced monitoring inspection was conducted on July 26, 2018 and completed on August 7, 2018 in regards to a self-reported incident regarding supervision. An inspection was conducted at the facility, interviews were held with staff and collateral witnesses and documentation was obtained. The following areas of standards were reviewed: Administration, Staffing and Supervision, Staff Qualifications and Training, Background Checks and Licensure and Registration Procedures. Areas of noncompliance are identified on the violation notice. The results of this inspection will be reviewed with the Licensing Administrator.

Violations:
Standard #: 22VAC40-185-340-A
Description: Based on staff interview and a review of documentation, the center did not ensure the care, protection and guidance of at least one child in care. Evidence: During the inspection conducted on June 29, 2018, a 14 month old infant reportedly wandered out of the Infant-Two Classroom on June 27, 2018 and was found in the hallway by a non-staff member approximately 50 feet away from the classroom and approximately 20 feet away from the main entrance of the facility. 1. The staff members and the Program Director reported that the child had been gone for "less than five minutes". 2. One staff member in the classroom was changing diapers at the time of the incident. The second staff member, according to center documentation and staff and collateral interviews, was in the hallway talking to another staff person while preparing to put a group of children in a stroller to take them on a walk. 3. The Program Director verified that after the non-staff member found the child and returned the child to the office, she came around the corner and saw Staff #2 talking with another staff person in the hallway and that the children "were scattered all over the hall". 4. Both staff members were unaware that the child was missing from their group until the Program Director came into the classroom to ask them "how many children they had".

Plan of Correction: There is zero tolerance and the staff was terminated that day. We will have a staff meeting that will incorporate a supervision training on Tuesday, August 14, 2018. All staff will know if this supervision policy is broken, it is grounds for immediate termination. On June 27th, I had already informed all of my staff that they are here for the children and are not to socialize. the only time that they should be out of their classroom is from one transition to another and not to randomly chat and talk. I will reiterate this at the meeting on August 14th.

Standard #: 22VAC40-185-340-F
Description: Based on observation, staff interviews and a review of documentation, the licensee did not ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff. Evidence: During the inspection conducted on June 29, 2018, it was verified through center documentation and staff and collateral interviews, that a 14 month old child was left out of sight and sound supervision of center staff for an undetermined amount of time on June 27, 2018 around 4:30pm. Multiple staff and collateral interviews confirmed that the child had been gone for "less than five minutes".

Plan of Correction: There is zero tolerance and the staff was terminated that day. We will have a staff meeting that will incorporate a supervision training on Tuesday, August 14, 2018. All staff will know if this supervision policy is broken, it is grounds for immediate termination.

Standard #: 63.2-1720.1-B-2
Description: Based on record review, in one of two staff records reviewed, the licensee did not ensure that staff submit to fingerprinting and receive results prior to employment. Evidence: During the inspection conducted on June 29, 2018, Staff #2 was hired on 3/14/2018 and the fingerprint results were dated 4/13/18.

Plan of Correction: Moving forward I will absolutely ensure that that background check is back prior to them being employed. That was a misunderstanding on my behalf.
