Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted from 10:30 am - 2:30 pm with the center owner director and assistant director. A licensing inspection was also conducted on this date (see that report for details). All staff have documentation of the mandatory VA Preservice Training and are currently certified in first aid and CPR. There are no children with diagnosed food allergies currently enrolled, and no medications on site. The emergency preparedness plan was reviewed for reunification plans and lockdown procedures. Standard #: VENDSUB-000-031-A
Description: Based on review of the emergency preparedness plan, the facility failed to have all components required for the emergency preparedness plan. Evidence: There were no written procedures on the reunification of children with parents after a lock down drill.

Plan of Correction: Am currently updating the emergency preparedness plan and I will add reunification procedures to the plan.

Standard #: VENDSUB-000-032-A
Description: Based on observation of the emergency response drill log, and an interview with the director, emergency response drills were not practiced as required. Evidence: there is no documentation that a lock down drill was conducted in the past year.

Plan of Correction: Am updating the lock down procedures and will conduct a practice drill when they are completed.
