Comments:
An unannounced monitoring inspection was conducted from 11:35am until 2:15pm. At the time of the tour, there were 78 children in care with 10 staff present. A sample of 8 children's records and 8 staff records were reviewed. Children were observed eating lunch, resting quietly during nap time, playing outside and participating in afternoon activities. Hand washing and restroom procedures were also observed. Medication, the emergency preparedness plan, documentation of emergency practice drills, children's incident reports and center postings were reviewed. One violation in the area of physical plant is identified on the violation notice and was discussed with the owner/director during the exit interview.

Description: Based on playground observation and measurement, the center did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards . Evidence: The mulch at the base of the two slides on the playground measured approximately one inch in diameter, where 6 inches are required.

Plan of Correction: The center's owner will ensure that the mulch on the playground is turned over and raked. Additional mulch will be purchased as needed throughout the year.
