- Technical Assistance:
Had a discussing with center management regarding hand washing and transportation procedures.
Today an unannounced Monitoring Inspection took place between the hours of 9:30am and 3:50pm. At the start of the inspection there were a total of 127 children with 24 staff; all ratios were incompliance. A total of 5 children?s files and 6 staff files were reviewed. The children were observed playing outside in sprinklers, playing with play doh, singing songs and other organized activities. The teachers outside were observed using walkie talkies to interact with inside management when assistance was needed. The summer campers were observed leaving on a field trip to a local pool. Standard #: 22VAC40-185-130-A Description: Based on record review, the facility did not obtain documentation of immunizations before a child attend the center. Evidence: Child #1 file did not contain documentation of immunizations. Plan of Correction: We will double check immunization records on health forms moving forward. I will contact the parents today to ask for this missing information.
Standard #: 22VAC40-185-60-A Description: Based on record review, children's records did not contain all required information. Evidence: Child #1 file did not contain two emergency contacts. Child #2 file did not contain the addresses for both emergency contacts. Child #3 file did not contain a complete street address for their second emergency contact. Child #4 and Child #5 file's did not contain complete street addresses for both emergency contacts. Plan of Correction: We will ensure the document requires an address for two emergency contacts. We will review forms for completion upon enrollment.
Standard #: 22VAC40-185-400-B Description: Based on interview and a review of the center's video recording, a staff member did not use behavioral guidance that was constructive in nature and resolved conflict. Evidence: On 7/18/2018, center management had self-reported an incident to the licensing inspector regarding a teacher that had swatted a child on the head. Video recording from the incident shows Staff #6 sitting on the floor of the Beginner A classroom surrounded by a group of children and applying sunscreen to Child #8. Child #9 was observed bouncing around in the group and hit Child #8 on the head. Staff #6's reaction was to swat Child #9 in the forehead. Staff #6 self-reported the incident to center management. Plan of Correction: After investigating the report, this staff member has been terminated. We are currently retraining all staff on child guidance policies.
Standard #: 22VAC40-185-490-E Description: Based on observation, contaminated surfaces were not cleaned and sanitized. Evidence: In the Toddler A classroom, Child #6 was observed drinking from their sippy cup. Child #6 then handed their sippy cup to Child #7. Child #7 then began to drink out of the sippy cup. The teacher observed the exchange and quickly removed the sippy cup from Child #7's mouth. The teacher then handed the sippy cup back to Child #6 without cleaning or sanitizing the cup. Plan of Correction: We will remind teachers that when children share cups that we need to thoroughly wash the cups before returning them back to the children.
Standard #: 22VAC40-185-510-C Description: Based on observation, the facility did not follow policies to ensure that expired medication could not be administered. Evidence: Child #2 has emergency medication on site. The medication expired 3/2018. Plan of Correction: We will contact the parents today to request medication that is not expired. We will send home the expired medication.
Standard #: 22VAC40-185-510-N Description: Based on observation, medication with an expired authorization was not returned to a parent or discarded within 14 days. Evidence: Child #4's medication authorization had expired on 7/24/2018. The medication was still on site at the time of the inspection. Plan of Correction: We will reach out to the parents to have the doctor fill out a new form. Moving forward, we will review beginning and ending dates on medication forms more closely.
Standard #: 63.2-1720.1-B-3 Description: Based on record review and interview, the facility did not submit an out-of-state background check for one employee that had lived outside of the state of Virginia within the last five years. Evidence: Staff #2 sworn disclosure statement confirmed that they had lived outside of the state of Virginia within the last five years. Staff #2 file did not contain documentation that a central registry background check had been submitted for that state. Center management confirmed that if a photo copy of the check is not in the file then it must not have been completed. Plan of Correction: We will get it filled out and sent out this week.
