22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today from approximately 11:30am-2pm. There were 67 children in the direct care and supervision of 12 staff. A sample of 7 children?s records and 5 staff records were reviewed. The children were observed playing on the playground, singing, reading, eating lunch and preparing for nap. Zero new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #: 22VAC40-185-160-C Description: Based on record review and interview with staff, each staff member did not submit follow up TB results at least every two years from the initial screening or testing. Evidence: Staff A completed her last tuberculosis exam 1/27/2015. Plan of Correction: She will go for a screening next week.
Standard #: 22VAC40-185-270-A Description: Based on observation, equipment of the center was not maintained in a safe and operable condition. Evidence: The red paint on two metal cars located on the playground was chipped and peeling. Plan of Correction: I will let the owner know and he will paint the playground equipment.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: 1) In the "Lil Montessori" room, cleaning materials were found unlocked on a high shelf in the bathroom. 2) In the "Toddler" room, cleaning and disinfectant materials were found unlocked on a high shelf in the bathroom and in the cabinet of the changing table. Plan of Correction: We will get locks for the cabinets before the end of the month.
Standard #: 22VAC40-185-550-D Description: Based on record review and interview with staff, the center failed to implement a monthly practice of evacuation. Evidence: An evacuation drill was not practiced for the month of January 2018. Director stated that the cold weather prevented them from conducting a practice evacuation drill for the month of January. Plan of Correction: All other drills have been practiced. Going forward we will conduct evacuation drills during the winter months.
