Health and Safety inspection was conducted from 3:15pm to 5pm to review supplemental health and safety requirements. There were 14 children in care with two staff. Children present ranged in age from 3 to 11years old. Children were observed in teacher and self-directed activities. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-005-B Description: Based on record review and interview the vendor failed to maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: 1 - Between 4:30pm and 5pm four children left the center with four different adults as it was the end of the day. However, the departure of each of the four children was not documented as it occurred as required in the vendor agreement. 2 - Staff 1 confirmed that the departure of the four children had not been documented as it occurred. Plan of Correction: Center management will ensure that the arrival and departure of children in care is documented at the arrivals and departures occur.

The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.Virginia Quality is a voluntary quality rating and improvement system for early care and education facilities serving children ages birth through pre-K.
