Comments:
An unannounced monitoring inspection was completed today. The physical plant, 5 staff files, 5 children's files, annual fire and health inspection, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected between 1:30-3pm. There were 30 children under the supervision of 4 staff. Today, the children had a field trip to One Loudoun, enjoyed an ice cream party, played with games and puzzles and resting during naptime. For lunch, the children were served macaroni and cheese, green beans pineapple and milk. The interactions between the children and staff were seen to be positive and constructive in nature. Description: Based on file review, 1 out of 5 staff records does not have a TB test/screening completed no later than 21 days of employment. Evidence 1. Staff #2 began employment on 6/19/18. There is no documentation on file of a TB test/screening having been completed.

Description: Based on file review, 1 out of 5 staff records does not have a TB test/screening completed within the past two years. Evidence 1. Staff #5's most recent TB test/screening was completed on 2/10/15.

Description: Based on file review, 1 out of 5 staff records does not have all required information. Evidence 1. Staff #5 does not have documentation that two or more references as to character and reputation as well as competency were checked before employment. There is only one reference check on file.

Description: Based on observations, there was an area in the center that was not maintained in a safe condition. Evidence 1. In the 3's classroom, there is a section on the window ledge where the paint is chipping off.

Plan of Correction: Areas of the center are being repainted currently. This task will be added to that.
