22VAC40-185 STAFFING AND SUPERVISION.
An investigation was conducted in response to the receipt of a self reported incident being received in our office. Based upon multiple staff interviews and a review of documentation, the allegation of a lack of supervision is confirmed. See the Violation Notice for further information. Standard #: 22VAC40-185-40-E Description: Based upon multiple staff interviews, it was determined that staff failed to follow the policy for knowing where children are at all times when a child was left unattended for approximately 5 minutes. Evidence: The Sports Cabin 1 group left the cafeteria after lunch to go to the gym without collecting the accountability passports from the children. When the group changed location to the classroom, they were unaware that one child was not with the group. The staff still had not collected the passports when the child was returned to the group. Additionally, staff were not aware that a different child in the group did not have a passport. The system for accountability was not properly used. Plan of Correction: The policy was reiterated to all staff to make sure they use 4count accountability (Accountability system) to maintain supervision of children.
Standard #: 22VAC40-185-340-F Description: Based upon multiple staff interviews, the center failed to keep a child under the age of 10 in actual sight and sound supervision. Evidence: On Thursday August 2, 2018 a 7 year old child (Child A) (Date of Birth 11/14/11) was found, unattended at approximately 1:00 PM, in the hallway of the facility by a staff person. The child stated he had gone to the bathroom in the gym, and when he came out of the bathroom his group (Sports Cabin1) was gone. The group had entered the gym after lunch; however, staff changed their mind about where they should be headed, and left the gym to go to the Sports Cabin room. The child was without supervision for approximately 5 minutes. Plan of Correction: The policy was reiterated to all staff to make sure they use 4count accountability (Accountability system) to maintain supervision of children.
