Comments:
An unannounced renewal inspection was conducted on August 21, 2018, between the hours of approximately 10:25 a.m. and 3:45 p.m. Sixty one (61) children were enrolled and twenty-three (23) children, in four classrooms, were in the care of five staff members during the on site inspection. Two additional staff members were present and able to assist, as needed. At 3:30 p.m., eleven (11) school-age children arrived for care. Five children's records were reviewed and five staff records were reviewed and/or updated. Five children's medications, three children's topical ointments and the accompanying authorization forms/logs were reviewed. Safety equipment (first aid kit, flashlight, electrical outlet covers and radio), posted information (license, inspection documentation, weekly menu, emergency evacuation diagrams/plan, emergency telephone numbers and classroom schedules) and available documents (emergency evacuation drill/shelter-in-place drill records, attendance records and injury reports) were observed/reviewed. The daily schedules accurately reflected the activities provided. The lunch served on this date included: chicken patty on bun; broccoli; pineapple; and milk. Observed staff/child interactions were positive and supportive of children's development. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Program observation was completed. Activities observed on this date included: indoor play (toys, matching game and playdoh); books/stories; restroom, diapering and handwashing procedures; lunch; infant care; naptime and arrival/departure of children. Previously cited violations were reviewed for compliance; no repeat violations were cited. The exception to compliance was noted on the current violation notice. Description: Based on review of five staff records and discussion with staff on August 21, 2018, updated tuberculosis (TB) screening/exam results were not obtained/submitted at least every two years from the date of the first initial screening or testing for two staff members. Evidence: 1. Review/update of staff #1's record, on this date, determined that a current TB exam/screening report was not on file. 2. Review/update of staff #2's record, on this date, determined that a current TB exam/screening report was not on file. 3. Staff #3 confirmed the lack of current TB exam/screening results for staff #1 and staff #2.

Plan of Correction: Staff members will obtain/submit the results of current TB exams/screenings.
