Technical Assistance:
Licensing inspector discussed expired TB.

Comments:
An unannounced renewal inspection was conducted today from approximately 1:30pm-4:30pm. There were 122 children in the direct care and supervision of 19 staff. A sample of 10 children?s records and 5 staff records were reviewed. The children were observed engaged with staff, watching a movie, napping and during diapering procedures. Three new staff members have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, each child did not have a physical examination on file. Evidence: Child D did not have a physical examination on file.

Plan of Correction: Child is scheduled for 6 month check up appointment on Friday. Parents will send in documents once it is complete.

Standard #: 22VAC40-185-60-A
Description: Based on record reviewed, children's records were incomplete. Evidence: 1 of 10 records were missing required information. Child A was missing 2 of 2 emergency contacts.

Plan of Correction: Parents have just been relocated to this area and we will receive emergency contacts today.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: 1) In the "2's Room" a solution labeled "Bleach and Water" was found unlocked on the top of the diaper cubby in the bathroom. 2) In the "Preschool Room" toilet cleaner was found unlocked on top of a cabinet on the bathroom. 3) In the "Preschool Room" gel like air freshener was found on top of the toilet and accessible to children.

Plan of Correction: Corrected during inspection.

Description: Based on record review, staff did not request at least annually parent confirmation that the required information in the child's record is up to date. Evidence: 1) Child B's file was last reviewed 3/30/2015. 2) Child C's file was last reviewed 5/25/2016

Plan of Correction: Parents will confirm via email if there is any updates for the files.

Standard #: 22VAC40-185-550-M
Description: Based on record review, minor injury logs were incomplete. Evidence: 1) 2 minor injury logs were missing time parent was notified. 2) 1 minor injury log was missing the method of parent notification.

Plan of Correction: Staff will be informed to fill out form completely.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the most recent violation notice was not posted. Evidence: The violation notice posted was conducted on 08/08/2017. The last inspection occurred 03/29/2018.

Plan of Correction: Today's violation notice will be posted.
