22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted on 8/22/18 from 10:15 am until 2:30 pm. At the time of the inspection 58 children were in care with 13 staff. The sample size consisted of six children's records, six staff's records and three diaper ointments and supporting documents. Children and staff were observed during outdoor play, educational activities, art activities, free play, story time, clean-up, hand washing, snack, diapering, bottle feeding, lunch, nap, pick-up, transitions, and behavioral guidance. Standard #: 22VAC40-185-240-C Description: Based on record review and interview, one out of six center staff who work directly with children failed to annually attend at least 16 hours of staff development activities that are related to child safety and development and the function of the center. Evidence: 1. The record of staff 1 only contained documentation of two hours of training from staff member's start date of 7/25/17 to 7/24/18. 2. The director and staff 1 verified the two hours of training was all the training completed for that time frame. Plan of Correction: Staff 1 was advised to ensure she completes at least 16 hours of training for the time frame of 7/25/18-7/24/19 and provide documentation of the training for the record.
Standard #: 22VAC40-185-520-C Description: Based on interview, the center failed to obtain written parent authorization to administer diaper ointment for one out of three children. Evidence: The director was unable to find the written authorization form to administer diaper ointment for child 7. Plan of Correction: The parents of child 7 will be asked to complete a new diaper cream authorization form.
