Technical Assistance:

The licensing inspector conducted an unannounced monitoring inspection Friday, August 24, 2018 from approximately 12:15 PM until 4:00 PM.
The following ratios were observed with a total of 85 children present during the inspection:
Infants - 15 children with 4 staff
Toddlers - 8 children with 2 staff
Twos (Discovery Preschool) - 15 children with 2 staff
Threes (Preschool) - 13 children with 2 staff
Fours & Fives (Pre-K) - 16 children with 2 staff
School Age - 18 children with 2 staff

During the inspection, children were observed being held, rocked and fed, resting at nap time, having their diapers changed, taking bathroom breaks and washing their hands, working puzzles, building with blocks, playing outside and eating snack. Snack consisted of either crackers or pretzels and cheese. All areas of the center including classrooms, hallways, bathrooms, kitchen and outdoor playgrounds were inspected. The center was equipped with toys and supplies and items were available to the children.

The following information was reviewed: injury reports, injury prevention plan, daily attendance, daily sheets, infant feeding plans, menu, practiced emergency drill logs, emergency supplies, parent agreements and staff training information. Medication is administered and medication and medication authorization forms were reviewed. Last documented health inspection: 07/26/2018. Last documented fire inspection: 07/03/2018. Standard #: 22VAC40-185-140-A
Description: Based on the review of eight children's records, one child did not have documentation of a physical exam. Evidence: During the inspection, the licensing inspector observed the record for Child #5 (enrollment date 07/19/18) did not contain documentation of a physical examination by or under the direction of a physician. Each child is required to submit documentation of a physical exam prior to or within the first 30 days of attending the center.

Plan of Correction: Per director: We have already requested a copy of the physical exam. We will follow-up with the family and begin to ask families to submit a copy of the physical exam prior to the child attending the center.

Standard #: 22VAC40-185-60-A
Description: Based on the review of eight children's records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #3 (enrollment date 03/29/17) contained the name, address and phone number of one emergency contact. Two emergency contacts along with their information is required for each child enrolled in the center. The record for Child #3 was last reviewed 03/28/17. Information contained in the child's record is required to be reviewed and updated if needed by the parent at least on an annual basis.

Plan of Correction: Per director - We will give the family a form to update and submit the required information.

Standard #: 22VAC40-185-500-A
Description: Based on observation, one staff member did not wash her hands after each diaper change. Evidence: During the inspection, the licensing inspector observed a staff member change the diapers of two children in the Twos classroom. After the first diaper change, the staff member proceeded to pick up the next child and begin the diapering process without washing her own hands.

Plan of Correction: Per director - I will retrain the staff and continue to monitor staff to ensure the correct diapering process is being followed.
