Comments:
An unannounced monitoring inspection was conducted on August 15 2018 between 11:00am and 2:00pm. There were 60 children in the direct care of 15 staff members. Children were observed playing on the playground space, washing hands, having creative play, building, and participating in art. Staff were observed providing positive reinforcement, teaching children how to make friends, and being consistently engaged with the children. Standard #: 22VAC40-185-500-B
Description: Based on observation, a diapering surface was not nonabsorbent. Evidence: There was an approximate 2 inch rip in the outer material exposing the absorbent material underneath.

Plan of Correction: We will provide a new pad as soon as we get the delivery.
