Technical Assistance:
Discussed use zones for the playground- 6 feet on all sides of equipment.

Comments:
An unannounced monitoring inspection was conducted from 9:00 am-1:00. There were a total of 112 children with 20 staff members present. Children were observed during snack time, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, the center did not obtain documentation of a child's immunization before the child can attend the center. Evidence: Child #1, date of enrollment 8/27/18 was observed in the infant room, there is no immunization record on file.

Plan of Correction: The parent will bring in the immunization record.

Standard #: 22VAC40-185-130-B
Description: Based on record review, the center has not obtained documentation of additional immunization every 6 months for children under the age of two years. Evidence: Child # 2, date of birth 5/12/17 does not have updated immunizations. Child #2 was enrolled 8/21/17, the last documented immunization record was 11/17/17.

Plan of Correction: The parents will bring in the records.

Standard #: 22VAC40-185-270-A
Description: Areas and equipment of the center are not maintained in a safe condition. Evidence: The metal door trim in the Infant 4 classroom is pulling away from the door. This is the door leading to the playground. The broken area is located at the lower left corner of the door. The staff has taped the area but the metal trim has sharp edges. The staff has placed a crib in front of the door so that the toddlers can not access the broken area, but the crib is blocking the classroom exit.

Plan of Correction: A thick layer of cloth was placed over the trim so the sharp edges are covered. The crib was removed from the door. A work order will be placed with INOVA to repair the door.

Standard #: 22VAC40-185-500-B
Description: Based on observation, disposable diapers are not disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: The step on trash can in the Preschool #1 class was broken, the staff was using her hand to open the trash can to dispose of dirty diapers.

Plan of Correction: I will order a new one today.

Standard #: 22VAC40-185-520-C
Description: Based on observation and record review, the center has not obtained written parent authorization to apply diaper cream and diaper cream is not labeled. Evidence: Child # 3 in the toddler #1 class has diaper cream with no parent authorization to apply the cream. A diaper cream was observed in the Toddler #1 class with no child's name. Two diaper creams were observed in the Preschool #1 class with no child's name.

Plan of Correction: We will have the parent complete the form. We will label all diaper creams. The classrooms will be audited to ensure that all creams are labeled.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, a central registry finding results have not been received by end of the 30th day of employment. Evidence: Staff #1 was hired 4/20/18, the central registry finding was sent 5/8/18. The search was returned due to being incomplete, the search was mailed back. There is no documented date of the return search and there has been no follow up for this search. The staff was observed in the infant room today.

Plan of Correction: I have left an email message for central registry for the results.
