An unannounced renewal inspection was completed on 8/29/18. There were 20 children present with two staff supervising at arrival and an additional staff who arrives at 3:00 pm daily. The site director arrived during the inspection also. Five children's records and four staff records were reviewed. There were no medications for any children at the site. The inspector arrived at the school at 2:40 pm and departed at 5:00 pm. The children were observed at arrival, during snack, during homework time, during outside time, and during departures. There was discussion about the following with the site director: upcoming health inspection (by 9/20/18), a fire drill that needs to be completed and documented for August 2018.
Standard #: 22VAC40-185-60-A Description: Based on record review, the center failed to ensure that all required information was in each child's record. Evidence: 1. The record of child #5 had the father listed as not authorized to pick up the child. The father of child #5 was listed on the child's birth certificate. There was no appropriate legal paperwork on file to support the center not to releasing the child to the other parent. Plan of Correction: The site director will talk with the parent to obtain legal paperwork or remove the father's name from the paperwork of not allowed to pick up.
Standard #: 22VAC40-185-500-A Description: Based on observation and interview, the center failed to ensure that hand washing was practiced as required. Evidence: 1. All of the children were observed from the beginning of snack to the end. The children did not wash their hands or use disposable wipes after eating snack. Plan of Correction: The staff will make sure the children use wipes after eating snack.
Standard #: 22VAC40-185-560-F Description: Based on observation, the center failed to ensure that the requirements for providing snacks were met. Evidence: 1. The posted snack menu for snack provided by the program was observed and it listed pretzels and applesauce for the snack for today. The children were served fruit and grain bars as a snack. The snack menu was not changed to reflect the substitution of the food made. 2. The program did not provide a snack that followed the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). A fruit and grain bar was served. Two different components from the food groups are required to be served. For ages 6-12, the USDA requires 3/4 cup of fruit or vegetable and 1/2 cup of grain. There was less than 1/2 cup of fruit in the bars. The bar did not constitute a fruit serving. Plan of Correction: The menu will be changed to reflect the actual foods served. The site director will talk with staff about appropriate snacks to be served to meet guidelines.
