22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted from 9:11am-1:00pm. At the time of entrance, 73 children were in care with 14 staff members present. Children were observed during circle time, play time, engaged in group activities, the monthly emergency drill and shelter in place drill. Positive interactions between the staff and children were observed in the infant A and infant B classroom along with the other classrooms. A selection of staff and children records and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not obtain a copy of a childs' physical examination within one month after attendance. Evidence: 1. Child #1 was enrolled on 7/09/2018 and the center did not have a copy of the child's physical.

Plan of Correction: I contacted the parent and they will be getting a copy of it to me next week.

Standard #: 22VAC40-185-160-C
Description: Based on record review, the center failed to obtain at least every two years from the date of the first initial screening or testing the results of a follow up tuberculosis screening for a staff member. Evidence: 1. Staff #3 had a tuberculosis screening results dated 8/10/2016 and has been more than 2 years from the last screening.

Plan of Correction: The staff member scheduled her tuberculosis screening for tomorrow.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center inside and outside were not maintained in a safe and operable condition. Evidence: 1. In the preschool classroom, a blue children's sofa had a few holes in the upholstery. 2. On the playground, a children's playhouse had a broken piece on top of the left table. 3. In the pre-k classroom, a children's play sink had a missing part that created a hole that could be a finger entrapment. 4. In the infant A classroom, the paint was chipping and peeling near five windows. 5. In the infant B classroom, a play mat was fraying on the edge and one corner creating a hole.

Plan of Correction: 1. I will put in another work order for the paint to be corrected on all 5 window sills. 2. I removed the broken piece off the playhouse immediately. 3. I will replace the sofa. 4. I will look at options to fill the spaces or will replace unit. 5. I will reach to the District leader to replace the infant play mat.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the center did not meet all hand washing requirements. Evidence: 1. In the 2 year old classroom, a teacher was observed changing a child's diaper and did not wash the child's hands once the diaper changing was completed.

Plan of Correction: I will retrain staff on hand washing procedures.
