Comments:
An unannounced renewal inspection was conducted on Friday, August 31, 2018 from 10:15am to 11:50am. The children were observed playing in learning centers, and then they went outside to play on the playground. The lunch menu and other required documents were posted. A formal observation was completed with this group. Five children?s records and two partial employee records were reviewed during this inspection. Medication is being administered and was reviewed. Transportation is provided, however the bus was off site during the inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 08/24/2018 Last shelter-in-place drill: 03/19/2018 Last fire inspection: 12/13/2017 Last health inspection: 05/2/2018 Total enrollment today: 16 Today, the following child to staff ratios were observed: Mixed Age Group Preschool (3-5 years old)- 16:2 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records on 08/31/2018, the center did not ensure that three of five records reviewed contained the required information. EVIDENCE: 1. The record of Child #1 did not have documentation of a complete address for the second emergency contact. 2. The records of Child #1, Child #2, and Child #5 did not have documentation of the first date of attendance.

Plan of Correction: Will let the case manager know what is missing in records.

Standard #: 22VAC40-185-290-3
Description: Based on observation on 08/31/2018, the center did not ensure that all electrical outlets shall have protective covers. EVIDENCE: The surge protector by the computer table had three uncovered outlets.

Plan of Correction: Corrected on site. Plug covers placed in outlets.
