Technical assistance was provided in the following areas of the standards: 22VAC40-191 (Background checks); 22VAC40-185-(3)-210 (Program Leader qualifications)

Comments:
An unannounced monitoring inspection was conducted on 8/28/18 from 9:45am - 12:30pm. At the time of entrance there were 45 children ages two years old through twelve years old in care with seven staff. Children were observed participating in various activities in the classrooms, swimming, departing for a field trip, eating lunch and during rest period. Records were reviewed for five children and five staff during the inspection. There is currently no medication at the facility. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a review of five staff records, it was determined that the facility did not ensure that each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment and shall have been completed within 12 months prior to or 21 days after employing. Evidence: 1. The record for staff #4 (date of hire 6/25/18) contained documentation of a negative tuberculosis screening that was dated 10/29/16. 2. Staff #6 reviewed the record for staff #4 and confirmed that the documentation of a negative tuberculosis screening was completed more than 12 months prior to employment.

Plan of Correction: The facility responded: From this date forward we will make sure all staff have TB within 21 days of employment. Staff #4 was that had expired. VP will monitor, Principal will ensure. (This applies to new employees only.)

Standard #: 22VAC40-185-580-I
Description: Based on observation and interviews, it was determined that the facility did not ensure that before leaving on a field trip, a schedule of the trip's events and locations shall be posted and visible at the center site. Evidence: 1. The Licensing Inspector observed a group of 12 children and two staff leaving for a field trip. When staff #1 was asked where they were going, she stated they were going to the local Library. However, there was no posted schedule of the trip's events and locations anywhere in the facility. 2. Staff #6 confirmed there no schedule of the trip's event and locations posted in the facility

Plan of Correction: The facility responded: All field trips are posted but the daily field trip was overlooked. (summer only). In summer (only0 we take field trips to Library. ABCCA will post all field trips & address & location. VP will monitor, Principal will ensure.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of five staff records, it was determined that the facility did not ensure that an employee is allowed to begin employment without a completed national criminal history record check (finger printing).

Evidence:
1. The record for staff #2 (date of hire 7/2/18), working during the inspection, did not contain documentation of a a completed national criminal history record check (finger printing).
2. The record for staff #3 (date of hire 7/12/18), working during the inspection, contained documentation of a a completed national criminal history record check (finger printing)
that was dated 8/1/18.
3. The record for staff #4 (date of hire 6/25/18), working during the inspection, contained documentation of a completed national criminal history record check (finger printing)
that was dated 7/2/18.
4. The record for staff #5(date of hire 6/21/18), working during the inspection, contained documentation of a completed national criminal history record check (finger printing) that was dated 8/17/18.
5. Staff #6 reviewed the record for the staff #2 and confirmed that she had not received the results of the finger print background check (finger printing). In addition, staff #6 also confirmed that staff #3, staff #4, and staff #5, all began employment prior to the facility receiving the results of their national criminal history record check (finger printing).

Plan of Correction: The facility responded: 1. We did not receive the 1st copy of CR findings until 8/30/18. We will make sure documents are in file before hire. Principal will ensure. 2. We will make sure CR is back before we start employment. VP will monitor, Principal will ensure. 3. Again, we make sure CR is back before we employ. 4. Same as #2 & 3 & 4. 5. Same as #2 & 3 & 4. We will make sure all employees receive finger printing before employment. VP will monitor, Principal will ensure.
