A Subsidy Health and Safety inspection was conducted on 8/28/18 from 9:45am to 12:30pm to review supplemental health and safety requirements. At the time of entrance there were 47 children ages two years old to twelve years old in care with seven staff. Children were observed participating in various activities in the classrooms, swimming, departing for a field trip, eating lunch, and during rest period. Records were reviewed for five children five staff . A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDSUB-000-004-F Description: Based on interviews and observation, it was determined that the vendor did not ensure to post , with parental approval, or keep in a way that is immediately accessible to staff, a current list of all children?s allergies, sensitivities, and dietary restrictions. Evidence: 1. There was not a posted list of all children?s allergies, sensitivities and dietary restrictions for the children enrolled at the facility. 2. Staff #6 confirmed that there was not a list of all children?s allergies, sensitivities and dietary restrictions for the children enrolled at the facility. Plan of Correction: The facility responded: This has been completed and the form we are using is enclosed VP will monitor, Principal will ensure.
Standard #: VENDSUB-000-005-B Description: Based on observation and interviews, it was determined that the vendor did not ensure that for each group of children, the vendor shall maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: 1. The Licensing Inspector reviewed the attendance records for the facility, and the attendance records did not have the arrival and departure documented for all children present. 2. Staff #6 confirmed that there was not a written record of daily attendance that documented the arrival and departure of each child in care as it occurs. Plan of Correction: The facility responded: Sign in sheets are in the front lobby along with the swiping machines (Sign in/out sheets & parent signature). We will have teachers monitor & have parents sign in & out daily. We keep roll books in every classroom. VP will monitor, Principal will ensure.
Standard #: VENDSUB-000-012-C Description: Based on a review of five staff records, it was determined that the vendor did not ensure that within the first 90 days of employment all staff shall complete the Virginia Preservice Training for Child Care Staff. Evidence: 1. The record for staff #1 (date of hire 9/6/16) did not include documentation to demonstrate complete of the all staff shall complete the Virginia Preservice Training for Child Care Staff. 2. Staff #6 reviewed the record for staff #1, and was unable to provide any documentation during the inspection to demonstrate completion of the Virginia Preservice Training for Child Care Staff. Plan of Correction: The facility responded: Staff #1 will have it completed in 10 days. All staff have completed VP training. VP will monitor, Principal will ensure.
Standard #: VENDSUB-000-012-E-1 Description: Based a review of five staff records, it was determined that the vendor did not ensure that all staff have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. Evidence: 1. The record for staff #2 (date of hire (7/2/18) did not contain documentation for a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. 2. The record for staff #3 (date of hire (7/12/18) did not contain documentation for a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. 3. The record for staff #5 (date of hire (6/21/18) did not contain documentation for a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. 4. Administrative confirmed there was not documentation for to demonstrate that staff #2, staff #3 and staff #5 had a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. Plan of Correction: The facility responded: Staff #2, 3, & 5 took 1st Aid & CPR 8/31/18 & have certification in their file. Staff #2 had already completed online training & had 1st Aid CPR for a number of years prior but has not physically present for the last training. VP will monitor, Principal will ensure.
Standard #: VENDSUB-000-012-E-2 Description: Based a review of five staff records, it was determined that the vendor did not ensure that all staff have a current certification in first aid appropriate to the age of children in care. Evidence: 1. The record for staff #2 (date of hire (7/2/18) did not contain documentation for a current certification in first aid appropriate to the age of children in care. 2. The record for staff #3 (date of hire (7/12/18) did not contain documentation for a current certification in first aid appropriate to the age of children in care. 3. The record for staff #5 (date of hire (6/21/18) did not contain documentation for a current certification in first aid appropriate to the age of children in care. 4. Administrative staff confirmed there was not documentation for to demonstrate that staff #2, staff #3 and staff #5 had a current certification in first aid appropriate to the age of children in care within 30 days of employment. Plan of Correction: The facility responded: Staff #2 did have a record of 1st Aid & CPR but did not have physical presence/she took online classes but did get hers 8/31/18 as well as staff 3, 4, 5. All employees are 1st aid CPR certified & current. VP will monitor, Principal will ensure. Note: Staff #2 was 1st Aid CPR trained online at the time of inspection & was current but not completed a class (in person) in person
