The Licensing Inspector conducted an unannounced monitoring inspection on 6/21/18 and 8/20/18 in response to a self-report made by the facility to the Licensing office on 6/20/18 about a child being injured while in care at the facility. Staff were interviewed, documentation was reviewed, and the entire facility was inspected and observed during the investigation. The evidence gathered during the investigation has resulted in a violation in the area of building maintenance, and is detailed on the attached violation notice. The violation was discussed with the Program Direction at the conclusion of the investigation.
Standard #: 22VAC40-185-270-A Description: Based on observation and interviews, it was determined that the facility did not ensure that all areas and equipment of the center shall be maintained in a safe and operable condition. Evidence: 1. On 6/20/18, child #1 got the index finger from her left hand smashed between the upper and lower sections of the bathroom door located inside the Two year old classroom. Child #1 received injuries to her index finger that required surgery to repair. 2. The bathroom door in the Two year old classroom appeared to originally be a single piece door , but was previously modified at some point to a door with an upper and lower section that were not connected, and worked independently of each other On the bottom of the upper section of the door, there were several areas that were sharp and protruding from the modification of the door that resulted in the injury to child #1's index finger. 3. On the upper door, there is a hook located on the back that is used to secure the upper door to the shelf in the bathroom so it did not move. However, the top section of the door was not secured to the shelf during this incident, therefore allowing the door to move back and forth freely. Since the door was not secured to the shelf, the upper door was able to close and cause child #1's left index finger to be smashed between the upper and lower door. Plan of Correction: The facility responded: On 6/30/18 we removed the half door and Licensing Inspector viewed the door that was removed. The door was never a single door since 1971, but we did remove the 1/2 door.
