Comments:
A Subsidy Health and Safety inspection was conducted on 9/6/18 to review supplemental health and safety requirements. Today's visit was a routine monitoring inspection. The inspection began at 11:00am and concluded at 3:30pm. There were nineteen children present between the ages of two and five years with five staff members, and thirty-two school age children present with five staff. During the inspection lunch time was observed, nap time was observed, required postings were viewed, emergency supplies were viewed, five medications along with authorizations were viewed, arrival of school age children was observed, one van used for transportation was viewed, seven out of sixty-five children's records were reviewed, and four out of fourteen staff records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on the inspector's review of four out of fourteen staff records, a staff member was no longer current in CPR certification. Evidence: The record for staff member #1 has CPR certification that had expired 0n 6/6/18.

Plan of Correction: The staff member is scheduled for CPR certification for next week.
