An unannounced investigation of a self-reported incident regarding forbidden action was conducted today. The inspector arrived 3:00 PM and departed at 4:00 PM. The investigation included review of documentation, viewing of a video recording and staff interviews. A violation resulted from this incident and can be found on the attached violation notice. An exit interview was conducted with the program director prior to closure of the inspection.
Standard #: 22VAC40-185-410-1 Description: Based upon review of documentation, review of video recording and staff interviews, the facility has not ensured that staff do not use forbidden actions. Evidence: 1. In a written statement provided, staff 1 sated that child 1, six years old, had tried to get a ball out of his hands and had thrown an "object" at him a "couple of times". Staff 1 wrote that, in order to get a ball back from child 1, he give child 1 "a little pinch on the arm". 2. A review of the video provided showed staff 1 interacting with child 1 who was tossing a small ball and a larger ball at staff 1. Child 1 was viewed in the corner of the gymnasium with other children when staff 1 went over to the group. At 5:16 PM., child 1 can be seen rubbing his arm as he walked away from the group in the corner. Plan of Correction: The facility responded with the following: Incident was self-reported to both CPS and licensing inspector immediately. Staff person was interviewed and terminated immediately same day of incident to ensure the safety of all campers. Staff will continue to be trained in behavior guidance.
