Technical Assistance:
Changing children in pull-ups was discussed with the program director. Those standards that apply to diaper changing also apply to changing pull-ups.

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 2:00 PM and departed at 3:45 PM. Seven children were in care with four staff. Nap time, snack time and afternoon activities were observed. It was reported that no medication is administered to children in care. Injury records were reviewed. A sample size of four staff records and five children's records were reviewed. An exit meeting was conducted with the program director prior to closure of the inspection.

Description: Based upon review of children's records and staff interview, the facility has not ensured that they obtain documentation of the required immunizations before the child can attend the facility. Evidence: 1. The record provided for child 2 indicates that the first date of attendance was April 16, 2018. 2. Staff 1 verified that the immunization record was not in child 2's record.

Plan of Correction: The facility responded with the following: The parent has be contacted to provided documentation of the child's immunizations. In the future, the requried health documentation will be carefully checked.
