An unannounced monitoring inspection was conducted on September 10, 2018 from approximately 9:20 am to 11:45 am. Three staff were present with fourteen children in the classrooms. Additional staff were on site for administrative and kitchen duties. Five children's records and five staff records were reviewed.
Standard #: 22VAC40-185-270-A Description: Based on observation, the facility did not ensure areas and equipment of the center are maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. Evidence: On September 10, 2018, the following was observed: 1. There is an approximate 4" x 5" area of peeling paint on the wall by the changing table in the preschool classroom. 2. There is rust on the metal fixtures under the sink in the preschool bathroom. Plan of Correction: The fixtures will be replaced within 7 days. The paint will be repaired within 7 days. Until the paint is done, a poster will be placed on the wall to cover the area.
Standard #: 22VAC40-185-280-B Description: Based on observation, facility did not ensure hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: On September 10, 2018, Lysol disinfectant wipes were stored in an unlocked cabinet under the sink in the preschool classroom. The container was labeled "caution...keep out of reach of children". Plan of Correction: The container was removed during inspection.
Standard #: 22VAC40-185-500-B Description: Based on observation, facility did not ensure the diapering area has a non-absorbent surface for changing children's diapers. Evidence: On September 10, 2018, there is a 1 1/2" tear in the diaper pad in the toddler classroom. Plan of Correction: The diaper pad was replaced during inspection.
