Comments:

An unannounced monitoring inspection was conducted on September 13, 2018. Eight staff were present with 41 children upon arrival to the facility. Additional staff were on site for administrative and support purposes. Children were observed in during music and movement, painting and doing small group table activities with their teachers that included cereal sorting, tracing and cutting and pasting. Infants were observed being fed, resting, having tummy time and sitting in swings. Five children's records and five staff records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, nutrition, injury report documentation, children and staff records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, liability insurance coverage, emergency supplies and procedures and required postings. Two medications were reviewed. The outdoor play area was not inspected due to inclement weather.

Standard #:	22VAC40-185-140-A
Description:	Based on record review, in one of five children's recors reviewed, the licensee did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: During the inspection conducted on September 13, 2018, there was no documentation of a physical examination in the record for Child #2 (first date of attendance 5/7/18).
Plan of Correction:	Physical will be requested from the parents today.
Standard #:	22VAC40-185-60-A
Description:	Based on record review, in three of five children's records reviewed, the licensee did not ensure that each child's record was maintained with all required information. Evidence: During the inspection conducted on September 123, 2018, the following information had not been documented in each child's record: Child #1-place of employment for both parents, work phone number for one parent and the names of any additional schools or programs that the child is concurrently attending Child #3 and Child #5-the names of any additional schools or programs that the child is concurrently attending
Plan of Correction:	New forms will be given to parents today. Emkployment information will be rectified today.
Standard #:	22VAC40-185-70-A
Description:	Based on record review, in three of five staff records reviewed, the licensee did not ensure that each staff record was documented with all required information. Evidence: During the inspection conducted on September 13, 2018, there was no documentation of job titles in the records for Staff #1, Staff #2 and Staff #3.
Plan of Correction:	Job titles will be rectified today.
Standard #:	22VAC40-185-440-J
Description:	Based on observation and inspection of the facility, the staff members did not ensure that there shall be at least thirty inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib. Evidence: During the inspection conducted on September 13, 2018, there was less than thirty inches of space on the access sides between two occupied cribs in the Infant room.
Plan of Correction:	Program Director will go over the standard again with staff to make sure that they are aware.
Standard #:	63.2-1720.1-B-2
Description:	Based on record review, in one of five staff records reviewed, the licensee did not ensure that staff submit to fingerprinting and obtain results prior to employment. Evidence: During the inspection conducted on September 13, 2018, Staff #1 received orientation training and was hired on 5/18/18 but the facility had not obtained fingerprint results until 5/25/18.
Plan of Correction:	An appeal will be submitted due to staff not having any contact with children until fingerprint was received on 5/25/18, as her start date was on 5/25/18.
