Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure all areas and equipment were maintained in a safe condition. Evidence: On the playground, the licensing inspector observed a cracked area of plastic with a sharp edge on a piece of equipment that was a safety concern for the children who would be using the equipment.

Plan of Correction: Per management, the sharp edge will be sanded down and repaired in a safe manner.

Description: Based on observation, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevented access by children. Evidence: During the inspection, the licensing inspector observed unlocked cleaning chemicals in a lower cabinet in the kitchen. The cabinet contained a lock, which was not locked during the inspection. The cleaning chemicals were moved to a locked location by a member of management during the inspection.

Plan of Correction: Per management, the center will immediately replace the lock on the kitchen cabinet which was not working previously.

Standard #: 22VAC40-185-320-B

Evidence: The licensing inspector observed that the two restrooms in the 4 to 5 year old classroom were not equipped with toilet paper during the inspection.

Plan of Correction: Corrected on site. Additionally, management will review policy with staff to ensure each restroom has necessary supplies each morning and again at nap time.
