An unannounced health and safety inspection was conducted today from 11am - 12 pm. There were 53 children present. All classrooms were within ratio. I observed children busy with creative activities including pasting and painting; singing; and preparing for lunch. Outdoor areas were not inspected due to weather. This inspection, along with the subsequent licensing inspection, meets the requirements for the SSHI inspection. If you have any questions, please contact me at firstname.lastname@example.org
Standard #: VENDSUB-000-006-B-11 Description: Based on a review of allergy action plans, a child with diagnosed allergies did not have a specific action plan as required. Evidence: Allergy medication was present for Child # 1, but no allergy action plans were accessible as required. Plan of Correction: We will give the forms to families today.
Standard #: VENDSUB-000-012-B Description: Based on a review of staff records today, there was no documentation of Virginia Preservice Training within 90 days of employment as required. Evidence: There was no documentation of Preservice Training for Staff # 2 (date of hire 5/18) and Staff # 3 (4/18). Plan of Correction: They are not returning to work until it is done.
