An unannounced monitoring inspection was conducted after school today. There are 3 staff with 15 children. 3 staff records were reviewed for specific items only, as the full record is reviewed at the main office on another date. Three children's records were reviewed. See violation notice form missing items. Children were observed washing hands, eating a cooking project (S'mores) and playing with a variety of activities. First aid supplies and emergency supplies are in compliance. There are no medications on site. Description: Based upon a review of records, one chid's record did not include documentation of an immunization record. Evidence: 1 of 4 records reviewed did not contain a shot record prior to the first day of attendance.

Description: Based upon a review of records, one child's record does not include a physical. Evidence: Child #1 does not have a physical on file.

Description: Based upon a review of records, one child's record does not include all required information. Evidence: Child #1's record lists an allergy. No allergy action plan is available for review.

Description: Based upon direct observation, the emergency evacuation maps posted do not include all required components. Evidence: The map posted does not show the primary or secondary evacuation routes out of the room in use.

Plan of Correction: The map will be corrected.

Description: Based upon a review of documentation, there is no documentation of monthly emergency evacuations drills. Evidence: There is no documentation available for drills in the months of March, April, May and June 2018.

Plan of Correction: Documentation is turned in at the end of the school year. I will maintain documentation for one year moving forward.
