Today an unannounced monitoring inspection was conducted starting at 2:20pm and ended at 4:00pm. There were 33 children present today with 3 staff. Children were observed arriving from school, washing their hands, eating snack (cinnamon roll and juice), and making cards for a staff member's last day. Three medications with authorizations were reviewed. Medication Administration Training (MAT) and First Aid/CPR certifications were checked today. Standard #: 22VAC40-185-130-C Description: Based on record review, the program did not obtain documentation of additional immunizations once between each child's fourth and sixth birthdays. Evidence: Child #2 is currently 11 years old. The most recent documented immunizations for this child are dated 10/7/09, when the child was 2 years old. Plan of Correction: I will speak to our office about reviewing health forms.
Standard #: 22VAC40-185-70-A Description: Based on record review, staff records did not contain all required information. Evidence: Staff #3 file did not contain a name, phone number or address for an emergency contact. Plan of Correction: I will contact our office to have them upload the information and I will ask for an additional copies
Standard #: 22VAC40-185-510-A Description: Based on a review of medications, authorizations and interview, the program has medication on site without written parental permission to administer the medication. Evidence: Child #1 has emergency medication on site for a medical condition. There is no written medication administration authorization from the parents. The program supervisor has confirmed that they do not have the written permission from the child's parents. Plan of Correction: I will have mom fill out the 10 day authorization and have the long-term form filled out by a doctor as soon as possible.
