Comments:
An unannounced monitoring inspection was conducted on 09/20/2018 from 10:20am - 1:20pm. At the time of entrance there were 16 children in care with four staff and the program director. The children ranged in age from 11 months - 5 years. Children were observed during outdoor play, lunch and nap. Records were reviewed for three staff and nine children. No medication is being administered at this time. The violations are listed on the violation notice and were reviewed with the program director at the conclusion of the inspection.

Standard #: 22VAC40-185-60-A
Description: Based on record review and interview, the center failed to ensure that children's records contain all of the required elements. Evidence: 1. The record for child 1 lacked a work phone number for the father. 2. The program director confirmed that the phone number was not on file.

Plan of Correction: The work phone number will be obtained today for the father of child 1.

Standard #: 22VAC40-185-70-A
Description: Based on record review and interview, the center failed to ensure that staff records contain written information to demonstrate they have the training required by the job position. Evidence: 1. Staff 1 has 10 hours of training documented toward the 24-hour program leader qualification. a. Staff 1 was observed working with a group of children as a program leader during the inspection. 2. The program director confirmed that all of the training certificates for staff 1 were not on file.

Plan of Correction: Staff 1 has received all of the program leader training. She attended several in-service training classes in the spring of 2018 that covered balanced mixed-age grouping in addition to health and safety and programming. The training document has been misplaced. Clear documentation of all of her training will be placed in her record by the beginning of next week.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that outside equipment of the center shall be maintained in a safe condition. Evidence: 1. An exposed screw was observed protruding from a broken metal bracket attached to the outdoor castle play structure. a. The screw had a sharp point and was accessible to the children walking on the wooden platform that had become detached from the metal bracket. b. Injury could result if a child were to step on this screw as it was at ground level. 2. One brick is missing from the bottom stair way of the playground exit door from the back of the facility. a. The missing brick poses a trip hazard.

Plan of Correction: The screw was removed during the inspection. The metal bracket will be repaired and reattached to the platform of the play structure. The land lord of the building has been asked several times to make this repair. He will be contacted today and asked to replace the missing brick.

Description: Based on observation, the center failed to ensure that when playground equipment is provided resilient surfacing shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Evidence: 1. Grass was observed in the fall zone in front of and behind the swings. Additionally, grass had overtaken the mulch beside the swing and in front of the rock climbing wall. a. Two large mounds of mulch was observed on the playground however it had not been spread into the fall zone area of the swings and rock climbing wall. 2. Children were observed playing on the swings and rock climbing wall during the inspection.

Plan of Correction: The mulch has been delivered and is on the playground however, with all of the recent weather events - the mulch has not been spread. The mulch will be spread over the weekend.
