The inspector conducted an unannounced mandated inspection from 10:30 until 12:40 p.m. There were 14 children in care with three staff members supervising at all times. Children were observed at circle time, at centers, washing hands, going to and eating lunch. It was reported that there are no medications being administered and/or on site. Two buses were inspected. The violations are identified on the violation notice. Please call your inspector at 276-698-9981 with any questions.

Standard #: 22VAC40-185-140-A Description: Based on record review, the center failed to obtain a physical examination within 30 days of enrollment. Evidence: 1. The inspector reviewed five children's records. One of five (#1) did not have a physical examination and the child has been enrolled for more than 30 days. Plan of Correction: Staff have requested the examination from the parent.
Standard #: 22VAC40-185-60-A Description: Based on record review, the center failed to have all the requried information in the children's files. Evidence: 1. The inspector reviewed five children's records. One of five records (#1) did not have an immunization on file before the child entered care. Plan of Correction: Staff have requested the information from the parent and also sent a letter 9/9/18.
