Comments:
An unannounced monitoring inspection was made on 9/19/18. There were 26 children present and two staff supervising. A third staff person arrived at approximately 4:30 pm. Five children's records were reviewed. Three staff records were reviewed on 9/24/18 and the inspection was completed with the child care director on that date. No medications were observed. The inspector arrived at the school at 3:00 pm and departed at 5:00 pm on 9/19/18. The inspection was completed on 9/24/18 after staff record review. The children were observed during snack, during homework time, during free play time in the gym, and during departures. There was discussion with the site director about the following: requirements for having emergency medications for food allergies (in addition to the allergy action plan), children's records - having work phone numbers for parents, ensuring cleaning supplies used for snack tables are locked when not in use.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, the center failed to ensure that all children had documentation of immunizations required by the State Board of Health before the child can attend the center. Evidence: 1. Child #2 whose first date of attendance was 9/15/18 did not have documentation of immunizations prior to attendance. There was no documentation of immunizations in the record of child #2. Child #2 was present during the inspection.

Plan of Correction: The site director or child care director will contact the parent to get a complete health record.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that each child's record contained documentation of all required information. Evidence: 1. The record of child #1 was missing documentation of a complete address for one of two emergency contacts for the child (sreet name documented but no street/house number). 2. The record of child #5 was missing documentation of a complete address for two of two emergency contacts for the child (city only documented for the address).

Plan of Correction: The site director will document all information in the children's records.

Standard #: 22VAC40-185-340-F
Description: Based on observation and interview, the center failed to ensure that children under 10 years of age were always within actual sight and sound supervision of staff. Evidence: 1. Child #3 who is nine years old was observed from time of the arrival of the inspector (3:00 pm) until 3:42 pm alone and unsupervised in the computer lab of the school. 2. The inspector asked the staff about the number of children marked present on the attendance versus the number of children actually present. The staff stated that child #3 was in the computer lab of the school which was on a different level of the location of the other children and staff. The staff stated that the child checked in with the staff at the program and then goes to the computer lab until later.

Plan of Correction: No child will be able to leave the group. Staff will keep all children in sight and sound at all times.
