An unannounced monitoring inspection was conducted on 9/21/18 from 10:05am - 1:15pm. When the Licensing Inspector arrived there were 91 children ages two years old to five years old in care with 19 staff. Children were observed participating in various activities in the classrooms, playing outside, and eating snack. Records were reviewed for nine children and nine staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Standard #: 22VAC40-185-130-A Description: Based on a review of nine children's records, it was determined that the facility did not ensure that the center obtains documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: 1. The record for child #1, present during the inspection, did not contain documentation of an immunization record. 2. The record for child #7, present during the inspection, did not contain documentation of an immunization record. 3. Staff #10 (Assistant Director) reviewed the record for child #1 and child #7, and confirmed there was no immunization record available for viewing during the inspection for either child. Plan of Correction: The facility responded: The parents of child #1 and child #7 will be asked to provide a copy of a current immunization record. All newly enrolled children will not start unless they have provided an immunization record.
Standard #: 22VAC40-185-140-A Description: Based on a review of five children's records, it was determined that the facility did not ensure that each child in attendance had a completed physical within one month of attendance. Evidence: 1. The record for child #2 (date of enrollment 1/3/18) did not contain a physical examination. 2. Staff #10 (Assistant Director) confirmed that the record for child #2 did not contain a physical examination. Plan of Correction: The facility responded: The parents of the child #2 will be asked to provide a copy of the physical. All new children will have documentation of a physical examination within 30 days of enrollment.
Standard #: 22VAC40-185-60-A Description: Based on a review of nine children's records and interview, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each child enrolled that contains all required information. Evidence 1. The record for child #1 did not contain documentation for viewing proof of the child's identity and age. 2. Staff #10 (Assistant Director) confirmed that the record for child #1 did not contain all of the required information. Plan of Correction: The facility responded: The parents of child #1 will be asked to complete the missing information. All newly enrolled children will have their records reviewed at enrollment to ensure they are complete.
Standard #: 22VAC40-191-60-C-2 Description: Based on a review of nine staff records, it was determined that the facility did not deny continued employment of a staff who did not have a search of the central registry finding within 30 days of employment. Evidence: 1. The record for staff #9 (date of hire 2/6/18) did not contain documentation of a completed search of the central registry finding. 2. Staff #1 (Program Director) reviewed the record for the staff #9, and confirmed that the search of the central registry finding has not been received. Plan of Correction: The facility responded: Staff #9 stated she had completed the form and gave it to the previous Director, but we have not been able to locate the completed form. A new search of the central registry for staff #9 will be completed and mailed. All new staff will complete upon employment. If it is not received within 30 days of employment, we will call the Virginia Department of Social Services to check the status of the request.
